Tieto zlo\u017eky s\u00fa umiestnen\u00e9 do kontextu zdravotn\u00e9ho syst\u00e9mu, ktor\u00fd sp\u00e1ja primerane organizovan\u00fd syst\u00e9m poskytovania zdravotn\u00edckych slu\u017eieb s komplement\u00e1rnymi komunitn\u00fdmi zdrojmi a politikami (Wagner et al. 2005). Model podporuje proakt\u00edvny pr\u00edstup pri podpore posilnenia postavenia pacientov a ich komunity. CCM prispieva k zlep\u0161ovaniu kvality starostlivosti o pacienta a zdravia a elimin\u00e1cii etnick\u00fdch a soci\u00e1lnych nerovnost\u00ed. Ka\u017ed\u00fa z dom\u00e9n je mo\u017en\u00e9 \u010falej aplikova\u0165 do strat\u00e9gi\u00ed, ktor\u00e9 s\u00fa nevyhnutn\u00e9 na dosiahnutie integrovanej starostlivosti. Roz\u0161\u00edren\u00e1 defin\u00edcia CCM v s\u00fa\u010dasnosti zah\u0155\u0148a aj \u0161ir\u0161ie spektrum determinantov zdravia ako je podpora zdravia, prevencia, skr\u00edning, personalizovan\u00fd mana\u017ement ochoren\u00ed, rehabilit\u00e1cia a paliat\u00edvnu starostlivos\u0165 (WHO, 2016).<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div
